Submitting a TSA Waiver Request Form
- Applicants must submit a completed TSA waiver request form to:
(571) 227-1945 (Primary Fax) or (571) 227-2948 (Secondary Fax)
- Waiver applications should be submitted no later than seven (7) days in advance of the flight. However, the Office of Airspace waivers will make every attempt to accommodate those waiver applications received with less than seven (7) days prior to the flight date.
- In Section II of the form, the applicant must provide his/her first and last name, date of birth, city, State, and country of birth, passport number, passport country of issuance, and/or social security number in order for the form to be processed.
- Individuals with a single name should be noted as "single name only" when completing a TSA waiver form.
- An addendum of information attached to the original waiver request form is permissible when listing multiple aircraft and/or pilot and crewmembers.
- The start and end dates of flight must be indicated on the waiver form.
- A flight itinerary must be provided for all, Fleet, Single Trip and No Transponder waivers.
- The waiver request form must be signed and dated by the company head, pilot, or equivalent designee for the waiver to be processed.
Modifications to an approved waiver are acceptable when adding and/or amending personnel, flight itinerary, and/or dates, provided a copy of the authorized waiver is faxed to the
Office of Airspace Waivers and a written letter that states the exact nature of the change to be made to the waiver is submitted.
